At Trinity Health Plan of Michigan, we care about our members. Our plans ensure members can seek care for an Annual Wellness Visit or In-Home Assessment that’s critical to healthy aging without concern over cost. Better yet, we reward our members for completing these visits through our Member Rewards Benefit program.

How can members earn a reward?  

Members can complete an Annual Wellness Visit or In-Home Assessment and in return get $50 added to their Flexible Benefit Card after completing the necessary attestations. It’s our way of saying thank you for taking care of their health!
