We are a hospital-sponsored Medicare Advantage organization that provides high-quality, compassionate health care at an affordable price. 

Our plan was designed by doctors to make care and coverage work better together so members get the most out of their Medicare benefits. 

Available through the Trinity Health healthcare system, you’ll find a Trinity Health-affiliated Medicare Advantage plan in select Connecticut, Idaho, Iowa, Michigan, New York and Ohio and counties.

Our products

Across our service regions, we offer Medicare HMO plans and Medicare PPO plans.

A Health Maintenance Organization (HMO) 

Our Medicare HMO plans cover in-network doctors and facilities, making them more affordable with lower premiums. We do not require referrals for specialists, allowing members to get the care they need without delays. Our plans also often don’t require pre-authorization for procedures recommended by network doctors, providing faster access to necessary treatments. 

With our HMO plans, members may be eligible for $0 premiums and cashback options. 

Preferred Provider Organization (PPO) 

With our PPO plans, members will enjoy many of the same benefits of our Medicare HMO plans with the added flexibility of being able to visit both in-network and out-of-network doctors and facilities.
