Pay My Premium


If you’ve selected a Saint Alphonsus Health Plan with a monthly premium, your payment must be received by the 10th day of each month. Paying your monthly premium is easy. You have three options to pay:

  • Electronic Funds Transfer. Your payment is automatically withdrawn from a designated bank account. To enroll in this payment method, please complete the Electronic Funds Transfer Form and mail to the address listed on the form. If you prefer to fill this out electronically, you may do so here: Online EFT Form.
  • Social Security Withdraw. Your payment is automatically withdrawn from your Social Security check. To enroll in this payment method, please complete the Social Security Withdraw Form and mail to the address listed on the form.
  • Billing Statements. Receive a monthly billing statement and have your check to Saint Alphonsus Health Plan by the 10th of each month.

You can change your payment method at any time. If you need assistance, please contact Member Services.

Payment Assistance

If you get extra help from Medicare to help pay for your Medicare prescription drug plan costs, your monthly plan premium will be lower than what it would be if you did not get extra help from Medicare.
