Saint Alphonsus Health Plan is available in select Idaho counties.
Select the name of the plan you are enrolled in to view your plan-specific documents — Summary of Benefits, Evidence of Coverage, and Annual Notice of Changes. You can find your plan name on your member ID card.

2024 Plan Consolidation
On January 1, 2024, we will combine Saint Alphonsus Health Plan Plus (HMO), H6910-003-001, with Saint Alphonsus Health Plan No Premium (HMO), H6910-003-000. The plan is now known as Saint Alphonsus Health Plan No Premium (HMO).
