Our Flexible Benefit Card is a convenient way to pay for qualifying expenses without having to draw from your own bank account. The prepaid card holds money that can be used like a debit card on out-of-pocket expenses associated with covered services under your insurance plan.

Our Flexible Benefit Card entitles you to an annual allowance towards plan-covered vision and hearing services. You may use it to supplement your vision hardware allowance, hearing aid co-pays, or to cover the cost of a covered hearing or vision exam.  

The Flexible Benefit Card also serves as a Member Rewards redemption card. These rewards are automatically loaded onto the card and are ready for use at qualifying retailers.

Making the most of our Flexible Benefit Card as a member

Call Member Services at 1-800-240-3851 (TTY 711) between 8 a.m. and 8 p.m.

Your card has already been activated with a vision and hearing allowance. Your Member Rewards will automatically load when earned during each calendar year.

Go to www.mybenefitscenter.com or download the OTC Network app from your App store to check your card balance and view eligible benefits and rewards. You can also call 1-833-684-8472 to check your card balance.

Your vision and hearing allowance balance is available at the start of enrollment or each plan year and will expire at the end of each plan year.
