Select the name of the plan you are enrolled in to view your plan-specific documents — Summary of Benefits, Evidence of Coverage, and Annual Notice of Changes. You can find your plan name on your member ID card.
2025 Plan Name Changes
Several of our plans will have a new name beginning Jan. 1, 2025:
- MediGold Cash Back No Premium (HMO) will change to Trinity Health Plan New York Cash Back (HMO)
- MediGold Cash Back No Premium MA Only (HMO) will change to Trinity Health Plan New York Glory No RX (HMO)
- MediGold No Premium (HMO) will change to Trinity Health Plan New York No Premium (HMO)
The menu below has been updated to reflect the 2025 plan names.

On Jan. 1, 2025, we will be combining MediGold Plus (HMO) with one of our plans, Trinity Health Plan New York No Premium (HMO). The information in your Annual Notice of Changes explains the differences between your current benefits in MediGold Plus (HMO) and the benefits you will have on Jan. 1, 2025, as a member of Trinity Health Plan New York No Premium (HMO). Unless you choose another plan, we will automatically enroll you in Trinity Health Plan New York No Premium (HMO) in 2025.

At any time, you can request a printed copy of the Provider/Pharmacy Directory by calling Member Services. You can also call to ask Member Services for more information about our network providers, including their qualifications and any changes in the pharmacy network.
